Community nursing care at home, not in a clinic
Community nursing brings a qualified nurse to your home so you can manage complex health needs without repeated trips to hospital or a GP clinic. Some people call it district nursing or in-home nursing; under the NDIS, it is funded as community nursing care.
Kuremara’s Brisbane nurses support NDIS participants who live with disability and need regular clinical care, from daily medication and catheter management to PEG feeding, tracheostomy care and palliative support. Every visit follows a nursing care plan we build with you, your family and your support team.

NDIS community nursing care under your plan
The NDIS funds nursing supports where the care relates to your disability and supports your daily living. These NDIS nursing services are delivered by registered or enrolled nurses and are usually funded from your plan’s Core supports (Assistance with Daily Life). Rates are set by the NDIS Pricing Arrangements.

What is community nursing?
Community nursing is clinical care delivered by a qualified nurse in your home or community rather than in a hospital. It includes tasks such as medication administration, catheter care, wound care, PEG feeding, and palliative support, guided by a nursing care plan.
Is community nursing covered by the NDIS?
Yes, where the nursing care relates to your disability and supports your daily living. It is funded as community nursing care in your NDIS plan. Health care unrelated to your disability is generally funded through the health system instead.
How do I get a referral for NDIS community nursing in Brisbane?
Your support coordinator, GP, or hospital discharge team can refer you, or you can contact Kuremara directly on 1300 000 799. We’ll check your plan funding and arrange a nursing assessment.
What's the difference between a community nurse and a support worker?
A community nurse is a registered or enrolled nurse who performs and oversees clinical tasks. Support workers help with daily living. Nurses can also train and support your workers to carry out some tasks safely under a care plan.
What can a community nurse do at home?
Our nurses provide injections, medication administration, PEG and enteral feeding, catheter management, complex bowel care, tracheostomy care, wound and pressure injury care, continence assessments and palliative care.
Can your nurses work alongside my current support provider?
Yes. Many participants keep their existing supports and add Kuremara Community Nursing. We coordinate with your other providers so everyone follows the same care plan.
