  • Health and other sensitive information: information about a disability, health condition, care needs, medication, behaviour support, cultural background or other matters relevant to safe and appropriate support.
  • Employment and recruitment information: information provided in an application or during onboarding, such as work history, qualifications, references, screening checks and training records.
  • Website information: your IP address, device and browser information, pages visited, form activity and information collected through cookies or similar technologies.

We aim to collect only information reasonably needed for our activities. We generally seek consent before collecting sensitive information, including health information, unless the law permits or requires us to collect it without consent.

2. How we collect information

We may collect information directly from you when you complete a form, call, email, speak with our team, apply for a role or receive our services.

Where appropriate, we may also receive information from a person acting on your behalf, a family member or carer, a support coordinator, a health professional, a hospital, another provider, a referee or a relevant government or NDIS body. We take reasonable steps to make you aware of the collection where required.

If you provide information about another person, please make sure you have authority to share it with us.

You may contact us without identifying yourself where this is practical. We will need enough information to respond to some enquiries and to assess or deliver support safely.

3. Why we collect and use information

We collect, hold and use personal information to:

  • respond to enquiries and communicate with you;
  • assess support needs and discuss suitable services;
  • plan, coordinate and deliver disability and health-related supports;
  • communicate with people involved in your support, where authorised or otherwise permitted by law;
  • manage bookings, service agreements, funding, claims and billing;
  • maintain records, respond to feedback and complaints, and manage incidents;
  • recruit, onboard and manage workers;
  • operate, secure and improve our website and services; and
  • meet our legal, regulatory and NDIS-related obligations.

We use health and other sensitive information for purposes connected with assessing and providing support, and for other purposes only where you have consented or the law allows or requires it.

We do not use your health information for direct marketing unless you have agreed to that use. If you choose to receive marketing communications from us, you can opt out by following the instructions in the message or contacting us.

4. Who we may share information with

We may disclose information where it is reasonably needed for the purposes described above, you have authorised the disclosure, or the law permits or requires it. Depending on the circumstances, recipients may include:

  • Kuremara staff, support workers and authorised contractors;
  • your nominated family members, carers, representatives or support coordinators;
  • health professionals, hospitals and other providers involved in your care or transition of support;
  • the NDIA, the NDIS Quality and Safeguards Commission, plan managers and other relevant bodies;
  • providers of systems we use to operate our services, such as website hosting, secure communications, recordkeeping and business software; and
  • regulators, emergency services or law enforcement agencies where disclosure is required or authorised by law.

We limit disclosures to information relevant to the recipient’s role and the reason for sharing it. We do not sell participant information.

5. Overseas access and disclosure

Some authorised administrative support for Kuremara may be provided from India. This may involve access to personal information where it is necessary for that team’s work. Some technology providers may also process or store information outside Australia.
